The Case Manager reports to the Program Director and provides services which will assist individuals in gaining access to needed medical, social, housing, educational, employment and other services directed by the Program Director and driven by the Personal Services Plan. 


  1. Assist in the accomplishment of goal directed objectives under the supervision of the Program Director by screening and evaluating specific Case Management activities as identified in the Parole Outpatient Clinic (POC) assessment and ISMIP Individual Service Plan. 

  2. Provide Mental Health and Substance Abuse counseling. 

  3. Maintain appropriate Case Management documentation as per the requirements of Case Management guidelines.

  4. Attend job-related conferences and also participate in a variety of in-service training and seminars.

  5. Attend regularly scheduled POC and staff meetings.

  6. Understand the basic concepts of mental illness and mental health including the physical, emotional and social needs of emotionally disturbed individuals.

  7. Provide effective and thorough feedback to the referring Parole Agent of Record for specific Case Management progress.

  8. Motivate clients to develop self-reliance in daily living.

  9. Monitor clients in the community and provide transportation to appointments and services.

  10. Provide groups on life skills and related topics.

  11. Think and act quickly and efficiently in emergencies.

  12. Comply with all Turning Point Safety policies and procedures, including but not limited to: workplace safety, reporting work related injuries, Infection Control and preventing potential safety risks for staff, clients and others.

  13. Will drive on Agency business.

  14. Punctual and regular attendance is an essential responsibility of each employee at Turning Point. Employees are expected to report to work as scheduled, on time and prepared to start working. Employees also are expected to remain at work for their entire work schedule. Late arrival, early departure or other absences from scheduled hours are disruptive and must be avoided.

  1. Bachelor's Degree in related field or Associate of Arts Degree in Psychology, Social Work, Counseling, Rehabilitation and equivalency of three (3) years working in similar or related field. 

  2. Ability to communicate effectively in written and spoken English.

  3. Ability to communicate effectively in written and spoken Spanish preferred. 

  4. Ability to effectively work with diverse ethnic, religious, socioeconomic and professional groups of people.

  5. Ability to plan, coordinate, and facilitate services for mentally ill clients.

  6. Current negative TB test and certified training in emergency first aid procedures including CPR.

  7. Ability to assist Mental Health Professionals in screening and identification activities.

  8. Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint.

  9. Ability to operate within Electronic Medical Records (EMR) system. 

  10. Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws.  Agency insurer authorization is required. 

  11. Ability to pass a criminal background check.

  12. Ability to pass a pre-employment physical, drug screen, general physical, and TB test.

  13. Must maintain good relationships with consumers, co-workers, government and civic representatives, as well as community members within whom the Agency is transacting business, and relate to them in a professional manner.
